ITEM: AUTHORIZATION TO UTILIZE AN EXISTING CONTRACT FOR THE FOREST HILL BOULEVARD CONFLICT STRUCTURE PROJECT
REQUEST: Authorization to utilize a previously awarded contract with D.S Eakins Construction Corporation, for the Forest Hill Boulevard conflict structure, in the amount of $85,000.00.

EXPLANATION: A sinkhole has recently developed in the traffic median on Forest Hill Boulevard and Montauk Drive. After further investigation, it was determined that the directional drill operation for the installation of the reuse water line had drilled through the storm sewer pipe, thus causing the sinkhole. The reuse water main project was completed several years ago. Upon discovery of the sink hole, the Public Works Department filled the sink hole and temporarily patched the road to “safe up” the area. The Engineering Department designed a conflict structure for the repair of the pipe and would like to complete the project as soon as possible to avoid additional damages.
On July 14, 2015, Council approved an annual contract (#007-15/DZ) with multiple vendors to provide various services including the installation of drainage pipe and structures. The initial term of the contract is for three (3) years, expiring on July 14, 2018. D.S. Eakins Construction Corporation was one of the awarded vendors. Staff has determined that this contract is the best option for the completion of the required repair and construction.
Staff is seeking authorization to D.S. Eakins Construction Corporation, the lowest priced awarded vendor, under their existing approved contract for the Forest Hill Boulevard conflict structure project at a cost of $71,104.00. The scope of work includes all labor, project site dewatering, equipment and operators, as well as maintenance of traffic (MOT), construction/ materials testing, precast structure, median repairs and the roadway repairs (base rock, asphalt mill & overlay and striping). D.S. Eakins Construction Corporation has an existing contract with Wellington that includes labor, project site dewatering, and equipment/operators, the remainder of the items are not contracted items, but are required to complete the project. The Engineering Department has reviewed the cost of all the non-contracted items and deemed them reasonable. Due to the possibility of unforeseen conditions, the Engineering Department recommends a contingency of approximately 20% ($13,896). The total project cost for the Forest Hill Boulevard conflict structure including contingency is estimated not to exceed $85,000.
